REFERRAL FORMS
Referral forms are accepted from health care providers, social workers, school staff, case manager/service coordinators, parents/guardians, among others!
Please ensure the parent/legal guardian of the child is aware and consented to the referral for services.
Families can also self-refer for services by e-mailing info@childrenstherapycollective.com or calling 204-202-3259.
Forms are available in electronically fillable or print versions. Please fax completed referral forms to 431-305-5704 or if parent/legal guardian has provided permission to e-mail it to info@childrenstherapycollective.com.
Once a referral form is received, we will reach out to the family to set up an appointment.
